Enter the number of days at which you wish to have a billing fee assessed if the amount of the service has not been paid

Minimum Fee

The amount of the billing fee is determined by the calculation of the Monthly Billing Fee Percent times the balance outstanding.  However if that amunt is less than the Minimum Fee amount, then the Minimum Fee Amount will be used.
