Problem: CLIA# error
Correction: CLIA is only required by Medicare or Medicaid, however you may get this on what is a
Medicare or Medicaid replacement plan.  You will need to setup this insurance as you would a Medicare or Medicaid plan in the Payer Group Table.   Keep the insurance company name but make changes to the Payer Claim Filing indicator in the Payer Groups table
 
Problem: CLM Segment 1 - Claim Submitter Identifier is Missing.
Correction: In electronic claims the office has chosen to pass the Chart Id to identify patients.
            Make sure patient has Chart Id, create new claim.  Or change to using Patient Id instead
            of Chart Id.
 
Problem: CPT Table: Drug Unit Price field must be entered for any procedure that uses a NDC (National
Drug Code).  This is a change from the older software which did an auto
calculation of Drug Unit Price.
 
Problem: Element SV103 (Unit or Basis for Measurement Code) is ‘1’. The minimum allowed
            length is 2.
Correction: Table CHC5010, column ISA11_InterchangeControlStandardIdentifier has an
            incorrect value, it should be ^.
 
Problem: Element HI02-02 has been already used.  Diagnosis Codes (primary and secondary) are
Expected to be unique within claim
Correction: Diagnosis code in the service line probably has a space at the end of it. Go to the
            Ledger Tab and reselect the Diagnosis code from the list.  Also make sure that in
            the Diagnosis  table the code does not have a space at the end of it.
 
Problem: Error 9001 (Log file not available)
Correction: Using SSMS Take database offline then bring back online
                    database\right mouse\tasks\Take Off Line
 
Problem: Facility missing in Box 32 on electronic claims (even though it prints on paper claim)
Correction: Edit the Patient Case and insure that a Facility is present - if not add it, then Create a new
Claim
 
Problem: Insurance Due / Private Due amount on the patient screen is wrong or not consistent when
            viewed from different computers.
 
Correction: The issue is that the parameters setup in statement printing needs to be modified.
The statement parameters are tied to each User and can be setup
differently for each User - this is by design so that statement printing etc can be
customized in offices with different providers/clinics etc. The balance on the patient
screen is a reflection of how the parameters are setup in the Statement printing screen
for each User. Therefore if you want all Users screen to reflect the same results
then every User must setup the Statement printing Parameters the same.
