Start Date

Enter/Select the date to start the Lookup

Last Name

Patient Last Name drop down box, click the drop down arrow to see a list of patient appointments

Patient Appointment

Select the patient whose appointments you wish to see

Individual Appointment

Select an individual patient appointment

Print

Click on the Print button to cause the appointment(s) to print

Day Appointments

Check mark the appointments you wish to see printed

Ok

Click the Ok button to proceed to the Print screen
